Late Registration for Spring 2012
A few options are available:
Either download a registration form below or pick one up at
15806 Clearcrest Dr.
Houston, 77059 (in Brookforest)
The form can either be
(1) mailed to the P.O. Box at the top of the form or
(2) faxed to 281-488-7566 or
(3) dropped in the clay pot by the front door at the Clearcrest
address.
If picked up or dropped at Clearcrest, there is a ziploc
bag containing blank forms (in the the clay pot).
Completed forms should also be placed in that bag. NOTE: If the
clay pot does not have an American flag in it, you are at the
wrong house.
If faxed, the original and the check should be mailed to
the P.O. Box. No cover sheet is needed on the fax.
